Spousal Sponsorship Documents Checklist

Official Document Checklist – Spouse (including dependent children of the spouse) [IMM 5533]

Sponsor’s Forms:

1. Application to Sponsor, Sponsorship Agreement, and Undertaking [IMM 1344]
2. Sponsorship Evaluation and Relationship Questionnaire [IMM 5532]
3.  Use of a Representative [IMM 5476]

The principal applicant must fill out these digital forms online:

1. Generic Application Form for Canada (IMM 0008)
2. Schedule A – Background/Declaration (IMM 5669) – you must fill out this form for yourself, and on behalf of any family members who are 18 or older
3. Additional Family Information (IMM 5406)
4. Supplementary Information – Your Travels (IMM 5562), if applicable. 
5.  Use of a Representative [IMM 5476], both the principal applicant and sponsor can sign.

1. Documents to support your status in Canada, 
2. Documents to support that you don’t currently have partners from previous relationships (if any),
3. Documents for your current employment/sources of income, 

1. Identity Documents, Travel Documents, and Passports
2. Civil Status Documents (marriage certificate, etc.)
3. Additional Information for Children (if applicable)
4. Police Certificates
5. Photos (Passport Size)
6. Documents to Prove ongoing relationship to sponsor,
7. Birth Certificates of the children you have together (if any)
8. [if needed] Maximum 20 photographs of the sponsor and principal applicant together AND OTHER DOCUMENTS SUCH AS RECOGNITION, FINANCIAL SUPPORT, PAST COHABITATION, ETC.
9. Your marriage story from the beginning until now.
